I've never had this before, for many months. However, with recent firmware (0.2.48) I've observed this:

twice in a row while attaching the same sysmoOCTSIM using the same cable attached to the same USB hub, ... that has litrally worked hundreds of times before.
As sysmocom also received a similar report on raspi a few days ago, there may be something wrong.
I removed one SIM card and re-plugged it, and it worked. Unforunately it currently also works with that SIM card re-inserted. Weird.

Might be related to the patch that resets with external power upon replugging, but i don't really see why or how. This requires a raw usb packet capture or live debugging (hard to do without knowing that it will fail and where to break), there is no way to guess what is going on. I've never seen it happen.
